Complete Division Entry Forms
Download and complete entry forms for each division and student. Verify all details carefully.

Multiple contestants can be entered per division form. Please list from Class G through Virtuoso

Accompanist Information
Accompanist Requests
Live accompaniment is required where accompaniment is written. Pre-recorded accompaniment is not permitted.
Studios may:
• Provide their own accompanist
• Use the contestant’s teacher
• Request a festival accompanist
Required by July 7, 2026
• Accompanist request
• Copy of music
• Non-refundable accompanist fee
Original scores must be provided for judge, contestant, and accompanist.
Festival Accompanist Fees

$75 per event if requested by July 7, 2026

$100 per event if requested after July 7, 2026
Includes 20 minutes rehearsal the preceding evening and audition time.

Submission Checklist
A studio submission is considered complete when:
✓ Studio registration is completed online
✓ Online payment has been submitted
✓ All required division entry forms are completed
✓ Entry forms are postmarked or received by May 29, 2026
✓ Accompanist request is submitted by July 7, 2026 (if applicable)
✓ Required supporting materials are included where applicable
Incomplete submissions may delay scheduling and incur a late fee per entry or missing information.
